49ers QB Alex Smith has been ruled out for Monday night’s game against the Bears, ESPN’s Chris Mortensen first reported.

Second-year QB Colin Kaepernick will get his first career start.

Smith suffered a concussion in last Sunday’s tie with the Rams. Kaepernick completed 11-of-17 passes for 117 yards after replacing Smith.

Kaepernick has 21 carries for 177 yards and three touchdowns on the ground this season. Smith returned to practiced late in the week, but was not cleared by doctors to play on Monday.

The Bears are also without their starting quarterback, as Jason Campbell will start for Jay Cutler (concussion).
